Dihydrocodeine and Ibuprofen — Safe Combo?

Dihydrocodeine and Ibuprofen — Safe Combo?

For most adults, yes — dihydrocodeine and ibuprofen can safely be taken together, and the combination is more effective than either drug alone.

How the combination works

Ibuprofen is an NSAID — it reduces inflammation at the site of injury. Dihydrocodeine is an opioid — it blocks pain signal transmission to the brain. Together they hit the pain from both ends.

Safe adult dosing

  • Ibuprofen: 400mg every 6–8 hours, maximum 1.2g (1,200mg) in 24 hours for over-the-counter use
  • Dihydrocodeine 30mg: one tablet every 4–6 hours, maximum 180mg in 24 hours

Take ibuprofen with food to protect your stomach.

When to avoid ibuprofen

  • History of stomach ulcers or GI bleeding
  • Kidney disease
  • Heart failure or uncontrolled high blood pressure
  • Asthma triggered by NSAIDs
  • Pregnancy (especially third trimester)
  • Already on blood thinners (warfarin, apixaban) without medical advice

If any of these apply, paracetamol is a safer partner for dihydrocodeine. See paracetamol with dihydrocodeine.

Can you take all three together?

Yes — paracetamol + ibuprofen + dihydrocodeine is the classic UK “triple stack” for severe acute pain after surgery or fractures. Stick to the maximum daily doses of each.
